We need to gather some information about you before your first visit with us. This information helps us stay on time with appointments, so you don't have to spend your time in our waiting room. Additionally, having your information beforehand allows us to prepare for your appointment. Knowing ahead of time ensures we can provide you with everything you might need to make your visit comfortable and streamlined.
Each patient at Iowa Sleep & TMJ deserves customized treatment; this information ahead of time helps us do that. Please fill out the new patient paperwork. It should only take you about 10-15 minutes to complete. Thank you for your time!
What to Expect at Your First Visit
At your first appointment, Dr. Pick will review your completed forms and discuss your symptoms and concerns. Whether you're seeking treatment for sleep apnea, TMJ disorder, or snoring, we'll take the time to understand your unique situation.
We may perform a comprehensive examination of your jaw, airway, and bite alignment. If you have previous sleep study results, please bring them to your appointment so we can review them together.
